WAIVER AND CONSENT*

I agree to the CCM Youth Volunteer Policy stated below.

Please read the following carefully before signing this waiver and consent form.

In order to volunteer without your parent, youth volunteers must be at least 16 years of age and must have parent/guardian consent prior to volunteering.

I understand that CCM is a Christian organization that operates based on Christian principles. Whereas we do not discriminate against anyone for their religious beliefs, we do openly demonstrate our faith through our operating principles, core values and actions.

I hereby give my consent for my minor child to be a CCM Volunteer. I agree to release CCM from any and all liability of any kind or nature as a result of my child’s volunteer participation. If my child needs medical care while participating in this program, I give CCM staff or leadership permission to obtain appropriate treatment if rendered necessary. I understand that I shall be responsible for any costs incurred for this treatment.
