How to Receive Financial Assistance

You must have exhausted all other available resources before you can be considered for assistance. Please check your email for an application confirmation or denial.

It will be REQUIRED that you provide:

• Your past due rent notice or utility bill
• Your picture ID
• Documents confirming your current address
• Documents showing income and expenses for the current and previous months – i.e. pay stubs, receipts and bank statements for all adults in the home

Please note that, if approved for assistance, the maximum amount that CCM can help with is up to $500 once every 2 years.  If you need more than $500, you will need to pay the remainder balance before you can receive our funding.
