If you do not have a Shopper ID and PIN, or have forgotten it, you will need to place your order in-person at the pantry.  We will provide you with your Shopper ID card and PIN on your visit.  We encourage you to keep the card in you wallet, or take a photo and save on your phone.

The CCM Crisis Center Food Pantry is located at 246 Country Club Dr NE in Concord.  

Hours are Monday – Friday 10am -1pm.  Also Thursdays, 3:30-6pm for working clients.

No documentation or appointment is necessary.  It is a drive-through pantry, so you do not need to get out of your vehicle.  Walks-ups are welcome, too!

The completed AD-3027 form or letter must be submitted to USDA by:
